o Download the official life certificate form PDF from

2.  Personal Details

o Fill in your full name as per your pension records.

o Enter your Pension Payment Order (PPO) number accurately.

o Write your date of birth in the specified format.

3.  Bank Details

o Provide details of your bank branch where pension is credited (Branch name, account number, IFSC code if required).

o Specify your pension type (family pension or personal pension).

4.  Photograph

o Paste a recent passport-size photograph in the designated space if required by the form.

o Some forms may request the signature or thumb impression near the photo.

5.   Declaration/Certification Section

o The pensioner signs or puts their thumb impression confirming that they are alive.

o An authorized officer (bank official, gazetted officer, etc.) should attest the certificate with their signature, seal, and date.

o The officer may also add their designation and contact details.

6.  Verification of Life

o If submitting physically, present yourself before the authorized officer for attestation.

o For digital forms/Jeevan Pramaan, biometric authentication replaces physical certification.

7.   Submit the Form

o Submit the attested form to your bank branch or pension disbursing agency before the deadline.

o Keep a copy for your records.
